Financial Policies
Payment is expected
at the time services are rendered.
We accept cash, check, CareCredit, debit or
credit cards (Visa, MasterCard, and Discover)
as forms of payment.
We also accept insurance assignments as long as you are not restricted to a list of participating providers. Unfortunately, any Federal Employee insurance will not assign benefits to our office. Federal Employee payments are paid directly to the member. It is in the patient's best interest that the insured verify benefits before your first visit with our office.
As a courtesy to our patients, we are happy to submit insurance claims for you. All co-pays, deductibles, or non-covered services will be collected at time of service. Any charges not covered by your insurance will be your responsibility and a statement will be sent to notify you of any remaining balances. Once a balance has been carried in our office for 60 days, it becomes the patient's responsibility even if the claim is still pending with insurance.
CareCredit
CareCredit is a credit card offered in our office to help with dental and medical bills. It is a card that is accepted in many areas of business and will not be limited just in our office. CareCredit has multiple payment options to choose from, including some that are interest free. Sedation Fee
A $100.00 deposit is required for all sedation appointments.
24-Hour Notice
For any appointment we do require a 24-hour notice if a cancellation is necessary. Failure to provide an adequate notice of cancellation will result in a loss of your deposit or an additional fee for regularly scheduled appointments.
Medicaid
Our office is a participating provider for Hoosier Healthwise.
Delta Dental and Anthem Dental Blue
Our office is a participating provider with Delta Premier, Delta PPO, and Anthem Dental Blue (this does not include the Anthem Dental Blue Self employed plans). There are many types of Delta Dental and Anthem insurance plans so we encourage parents to call ahead to verify that we are on "their" particular insurance list. Being a participating provider means we voluntarily, as a courtesy to our patients, write-off an amount that is dictated to us by Delta Dental and Anthem Dental Blue.
